It is a consumable, restoring 20 Stamina and 30 Health.
You can get the achievement 'The Cake is a Lie' by attacking a guard with it.
It is also used in the breakout "Swimming with dolphins" at H.M.P Offshore; you have to craft a cake and give it to Snooty to escape.

The Cake is sometimes requested for in favours, and it can infrequently be found in desks or on knocked-out Inmates.
